1.  Pay your “pay to play” fee to the treasurer (located on the 2nd floor next to the main office).
2. Fill out the athletic forms and turn them in to Lisa Porter or Lisa Smith.

After you pay the fee and turn in your athletic forms, you will receive your blue clearance card.  Please fill out the back of the card and turn it in to Coach Orwig or Coach Kohuth.
